Exchange Teacher

I get to be a foreign exchange teacher next semester, in either Spain or Germany. I am concerned about a couple of things. Does anyone know if US medical insurance is honored in these countries or do I have to purchase International health insurance? Also, is my income tax free? I hope to continue to do some Freelance writing jobs while there as well, maybe even keep a blog journal. Has anyone done this before? I'm interested in tips and ideas.
